How do I schedule an appointment?
Please click on the request an appointment link above. You can either directly book an appointment or request for us to contact you in which case a member of our team will reach out to you for a brief phone call.
Once you’ve booked an appointment, you will then receive a link to our secure patient portal to complete intake paperwork, which includes consent and credit card authorization. Your initial appointment will be scheduled by our team after completion of intake paperwork.
You may schedule follow up appointments using our online portal (Osmind) once established as a patient.
Do you take insurance?
In order to provide you the quality of care that meets the standards we set for ourselves, we are unable to work with an insurance provider. Our practice manager will help you with the necessary documentation to apply for reimbursement from your insurance company. Please note, while we will provide applicable billing codes, the amount reimbursed will vary depending on your insurance plan.
While we are unable to take insurance, we do accept payments through FSA/HSA accounts.
When will my card be charged?
We require completion of consent and intake paperwork prior to scheduling your initial appointment. A $200 deposit is then charged to the credit/debit card provided to confirm the appointment. Upon completion of the appointment, the balance will be charged to the credit card/debit card on file.
What is your cancellation policy?
We understand that life happens and are here to work with you when those unforeseen events occur. In the event that you would like to reschedule a previously scheduled appointment, you may do so up-to 48 hours prior to your appointment time. Cancellations and reschedule requests within 48 hours of the appointment will be charged a $200 fee. Missed appointments will be billed at 100% of the session cost.
